For Rehabilitative/Restorative Services and Outpatient Therapy
The Brothers of Mercy Nursing and Rehabilitation Center and The Russell J. Salvatore Outpatient Therapy Center is a contracted provider with Medicare and most major insurance plans, including Highmark/Blue Cross, Independent Health, Univera, Aetna and United Healthcare.
We also participate with leading Western New York Accountable Care Organizations (ACOs), including:
- Agilon / Buffalo Medical Group
- Honest / Catholic Medical Partners
- Honest / Primary Care IPA
For individuals with Medicare who are enrolled in an ACO through their Primary Care Physician, a three-day hospital stay is not required before admission for short-term rehabilitation. This allows patients to transition more quickly to the appropriate level of care, reducing complications and supporting a faster recovery.
The Brothers of Mercy is the exclusive rehabilitation provider for the University Neurosurgery Group and Atlas Ambulatory Surgery Center. Patients who require rehabilitation following surgery can be admitted directly to our inpatient rehab center – bypassing a hospital stay and beginning physical and occupational therapy the same day.
For Certain Chronic Needs – Special Needs Plans (SNPs) are special insurance plans for qualifying individuals with severe or disabling chronic conditions, as specified by CMS. SNP type has its own eligibility criteria and benefits.
We are a participating provider in the following programs:
- Independent Health ISNP and CSNP– for members in assisted living, nursing homes, or in the community with specific diagnosis-related care needs
- United Healthcare ISNP / Optum– for United Healthcare members residing in a skilled nursing facility
For Long-Term Care:
- The Brothers of Mercy Nursing and Rehabilitation Center accepts Medicaid and most Long-Term Care insurances.
- Many Long-Term Care insurances will also cover a part or all of assisted living and memory care services at Sacred Heart Home.
